Safety and quality of care are at risk when a patient transitions between health care settings. Medication errors during these transitions can lead to patient harm and rehospitalizations. Including a pharmacist as a member of the patient’s health care team during discharges from nursing homes and home health services can decrease medication discrepancies, improve patient engagement and reduce hospital readmissions.
Amanda Schroepfer, PharmD, BCACP, will describe how to include a pharmacist as part of the health care team during these transitions and the goals of medication therapy management (MTM). Lyn Lais, RN, will share how MTM services can benefit patients during care transitions in home health services.

Amanda Schroepfer, PharmD, BCACP, Clinical Pharmacist, Goodrich Pharmacy
Amanda graduated with a doctorate in pharmacy from Creighton University in 2010. Then, she competed a PGY1 residency with the University of Minnesota and Goodrich Pharmacy. After residency, she was hired by Goodrich Pharmacy to help run their MTM program and staff their Anoka pharmacy location. In 2015, she achieved board certification in ambulatory care. Today, she continues to work at Goodrich Pharmacy where she is the Site Director for the residency program, continues to help run their MTM program, precept students and staffs at the Anoka location.
Lyn Lais, RN, Life Care Navigator, Lifesprk
Lyn believes educating the client and community is paramount to providing an easy transition of information between people, their providers, and care systems. She has previously worked at Methodist Hospital as a staff nurse and Walker Methodist Health Care Center in various leadership roles including Transition Manager. Lyn currently works as a Life Care Navigator at Lifesprk, a home health agency, which focuses on the holistic needs of seniors and helping them live longer, healthier and more independent lives.
